The Health Census provides an Excel spreadsheet of a group’s health, based on the questions answered on the specified application. The data is broken down on three separate tabs within the spreadsheet – one for dependent level information, one for employer level data, and one tab for the Group level.

The Health Census Report is designed to be viewed once all applications have been completed and signed for a group.
